Cornerstone Beach 41st - Outreach Coordinator (Referral Navigator) - The Child Center of NY

In this role you will:

• Coordinate with Clinical Coordinator/ Social Worker in supporting participants in identifying and creating a network of resources.


• Create opportunities to engage youth 14-24 years old to resources and various programing. Create bi-weekly programs/events for youth to build and established continued relationships.


• Conduct community outreach, build and establish relationships with community-based organizations CBOs; including working with community-based organizations (CBO’s).


• Coordinate group development incentives and activities, including meals, recreation, trips, cultural events, and other opportunities for connection and growth.


• Work with the local Cure Violence provider and other neighborhood resources to create a network of support for the participants.


• Engage participants regularly, encouraging participation and connection to the program.


• Update social medias of upcoming and past events for the Case Management Enhancement Program.


• Performs other related duties as assigned.

Position Type: Full Time

Hours: 11am to 7pm

What qualifications do you need:

• Bachelor’s Degree in Social Work or related field.


• 1 years’ experience in related field.


• Excellent communication and interpersonal skills and ability to work with diverse social, economic, and cultural groups.


• Ability to plan and carry out assignments independently.


• Ability to prioritize, adhere to timelines and multi-task.


• Proficiency in Microsoft Office.

• Bilingual in one or more of the following: Spanish and or Creole.
