Job Details
We are looking to hire an experienced Nurse Practitioner with active NY State APRN license with at least 2 years work experience, preferably 1 year working with Geriatrics to function as a Provider and provide direct skilled care to our healthplan members who are residents of assisted living and long term care facilities, as well as to those living at home in Manhattan.
In collaboration with physician, you will provide and coordinate primary care services to an assigned panel of Advantage members who reside in the community. You will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives
You will also act as clinical resource to the participating providers and facilities
You will act as team member and clinical resource to participating providers/facilities and to provide resource and contact for member/designated representative regarding all aspects of member individualized care plan and disease process
Work Schedule is Monday thru Friday
Qualifications:
Masters Degree in Nursing, graduate of accredited Nurse Practitioner program required
Previous clinical RN experience in acute, subacute and/or long term care setting preferred
Prior experience in manage care a plus
NYS NP License, Registration, DEA certificates required
