About the Job
The Sisters of St. Joseph, Albany Province are a community of women religious that operates out of the Provincial House in Latham. The person in this position is the care coordinator for Sisters of St. Joseph who reside at Teresian House. In this position, the Care Coordinator builds relationship between the individual sister, her health providers and the sister’s religious community to assure the care of the sisters.
DUTIES and RESPONSIBILITES
- Networks with the Staff at the facilities to assure the ongoing care of the sister. This includes Nursing, Social Services, Physical Therapy, Dietary, Scheduling, Business and Maintenance departments.
- Meets with the social workers and nurses for reports and changes in health care, coordination of the scheduling of appointments.
- Attends all sister Care Coordination Meetings quarterly.
- Provides on-call contact for Teresian House should there be a need for hospital emergencies. NOTE: Community Life Team will help with hospitalization needs.
- Works with Transportation Coordinator to provide transportation and escort services for medical appointments.
- Works with appropriate staff for the set up and removal of phone accounts and televisions.
- Works in partnership with the Community Life Director and Community Life Team to provide for meetings, communications and social/Holiday events.
- Oversees Community Life Team in setting up and removing personal items belonging to a sister.
- Oversees the Community Life Team members, with the Community Life Team Director, who are working at Teresian House.
- Reports directly to the Director of Community Life Team updates on scheduling and care meetings, health care changes etc.
QUALIFICATIONS and SKILLS
- Commitment to the mission of the Sisters of St. Joseph.
- An ability to confront with compassion and show respect for others, particularly the aging.
- Ability to live principles of accountability, subsidiarity and collaboration.
- Ability to demonstrate strong teamwork and support a positive work environment.
- Excellent communication and interpersonal skills.
- Ability to communicate private, sensitive and confidential information appropriately
- Ability to use a computer.
- Some understanding of the culture of women’s religious communities preferred.
- Experience in a field related to health care and/or pastoral care preferred.
- BA or MA preferred.
- NY State Driver’s license.
PHYSICAL DEMANDS and WORKING CONDITIONS
- Ability to travel to and from locations throughout New York State as needed.
- Ability to stand, sit, bend and walk throughout the day.
- Ability to see, hear, or use devices which makes these senses adequate to meet the requirements of the position.
- Ability to perform effectively with frequent interruptions.
- Willing to dress appropriately.
- Willing follow infection control mandates.
- Willing to be flexible in scheduling to meet the needs of the sisters and team.
Salary
20 - 30 /hour
