**General Purpose:**
The Center for Community Health & Prevention (CCHP) at the University of Rochester Medical Center (URMC) manages the Cancer Services Program of the Finger Lakes Region (CSP-FLR) and works collaboratively with the Wilmot Cancer Institute (WCI) to provide cancer prevention/cancer screening education and programs in the community.
Under supervision from the Program Manager, guidance from the Director of Community Health Partnerships (DCHP) and with latitude for independent judgment, the Community Liaison (CL) will work within the CSP-FLR and in collaboration with WCI and it’s Community Outreach and Engagement (COE) team.
In Monroe County and Livingston County, the CL will be responsible for creating and implementing the public education, training and outreach plan for the CSP-FLR. The CL will coordinate all outreach activities providing education and training to partners, providers and community members in order to reach the priority population. The CL will work closely with the CSP-FLR Outreach Manager in the Finger Lakes Region to ensure consistency in outreach efforts across the counties. The CL will also work closely with Wilmot Cancer Institute’s COE team to promote tobacco cessation programs and the benefits of lung cancer screening.
The Community Liaison will represent the CSP-FLR, WCI and the Center for Community Health & Prevention with partners, providers and patients and function in multiple capacities to assist in attaining all performance measures and program goals.
Flexibility required to work some evenings and weekends.
**Responsibilities:**
+ In collaboration with the Cancer Services Program Manager, manage and deploy outreach activities. Provide communication strategies and develop outreach plans to reach the priority population. Monitor metrics and evaluation processes, analyze and track results and trends for patient satisfaction and referral patterns, maintain compliance with HIPAA and PHI management. Maintain a working knowledge of cancer prevention and screening for all covered cancers (breast, cervical, colorectal and lung) including screening eligibility and healthy lifestyle prevention strategies. Stay abreast of the literature regarding cancer prevention, and participate in pertinent meetings and seminars.
+ Educate community partners about the importance of screening by attending community health-related talks and events, and networking with those in attendance to increase awareness of the CSP-FLR and WCI’s cancer prevention programs. Participate on community coalitions and establish and cultivate relationships with individuals and community-based organizations for partnership opportunities.
+ Responsible for partnering with community-based organizations, county health departments and other health networks in order to identify gaps in education regarding health and cancer prevention within the WCI catchment area. Collaborate closely with the Wilmot Community Outreach and Engagement (COE) team to identify new areas of need throughout the region. Attend Cancer Community Action Council (CCAC) meetings.
+ Key coordinator between providers and CSP-FLR to ensure protocols are followed for quality service delivery. Responsible for working with providers and clinicians to educate and train on the importance of cancer screenings and the services of CSP-FLR. Conduct periodic provider check-ins to replenish supplies, collect needed documentation and reports and ensure familiarity with CSP-FLR program and WCI cancer prevention initiatives.
+ Perform data entry and database management activities, enter program data accurately and timely, participate in evaluation design and resource allocation, monitor progress, and meet multiple deadlines on time. The CL will act as an ambassador of both CCHP and Wilmot COE programs, identifying new opportunities for engagement and/or growth or expansion of programmatic offerings. Work closely with both CSP-FLR and Wilmot COE for program deliverables and strategic engagement.
+ Support CCHP and WCI tobacco cessation efforts. Follow up with participants of the Commit to Quit program to provide a warm hand off to long-term lung cancer prevention services available within the continuum of care.
**Qualifications:**
**Required:**
+ Bachelor’s degree with major course work in an appropriate health, social or technical field.
+ 2-3 years of experience or an equivalent combination of education and experience.
**Preferred:**
+ Experience in nutrition, physical activity, or cancer prevention education a plus.
+ Ability to interact effectively with a wide range of individuals, from varying cultural, ethnic, socio-economic and educational levels.
+ Uses computer programs proficiently, including use of the Internet, Microsoft Word and Excel, and PowerPoint, database management systems, and data entry programs.
+ Ensures the security and confidentiality of all data per HIPAA, PHI and IRB guidelines.
+ Excellent interpersonal, presentation, and communication skills, including written communication.
+ Ability to work independently and as part of a team.
+ Excellent customer service, organizational and time management skills.
+ Some weekends and evenings are required.
+ Ability and willingness to travel.
