Education/Qualifications:
- RN. from an accredited school of nursing or college preferred.
- Current R.N. Rhode Island state license required.
- One year nursing experience in hospital, public health or community health preferred.
- Ability to perform duties and responsibilities across life cycle (infants, children, adolescents, adults, prenatal and geriatric) required
- Experience in providing managed care patient services. Commitment to work in community health with the challenges of a high risk, and often uninsured and diverse patient population required.
Essential Duties & Responsibilities:
- Assesses the immediate and long-term physical, mental and social health care needs of individuals and families.
- Incorporate age appropriate principles and consider the cultural and linguistic needs of patients and families to develop a plan of nursing care. Working with the individual and/or family to arrange follow-up care and provide patient education.
- Initiate action by direct contact, by telephone, or referral, according to approved guidelines.
- Case Manage families as needed to achieve the health care goals.
- Orient new patients to the Health Center and the delivery of care.
- Provide nursing services and treatments requiring specialized training, knowledge and skills such as immunizations, respiratory treatments, and medication delivery.
- Provide Nursing Visits to read PPDs, administer ordered immunizations and medications, and educate and counsel patients with acute/chronic health problem, for the promotion maintenance and restoration of health.
- Manage the flow of the clinic by previewing schedules and patient charts, proper triaging, and coordination and scheduling of sick visit appointments.
- Document all care delivered, all counseling, and all triage encounters by phone or walk-in. Demonstrate sound judgment as evidenced by documented recommendations to patients and timely communication with providers as warranted.
- Consult with Providers on the follow-up of DNKAs.
- Review all labs, and radiological test results. Chart all results (OB department). Document follow-up of abnormal results. Follow up on family/partner screening needs, i.e., TB, Hepatitis, STD, and Genetic Counseling.
- Maintain knowledge of programs and resources, i.e., WCSP, Diabetes Resource Center.
- Coordinate patient care with School Nurses, VNA, DCYF, Adult and Child Day Care, and Summer Camps.
- Maintain Pharmacy log including documentation of medications dispensed by providers and correct delivery and documentation by Pharmacy Representatives.
- Coordinate patient care by refilling prescriptions per policy for walk-in patients, patient phone calls and Pharmacist calls. Assist patients with medication delivery problem, i.e. fill weekly medication boxes.
- Coordinate and oversee Pharmacy Assistance Program
- Maintain medication logs, Depo Provera, Norplant, Oral Contraceptives.
- Check Emergency Cart and equipment as required.
