Community Health Worker

The Community Health Worker is responsible for identifying high risk prenatal and postpartum women and assist these individuals in gaining access to primary and preventive health services including prenatal care, PCAP, WIC, and other essential and supportive services as needed.
Responsibilities:
- Develop and maintain relationships with community leaders and community gatekeepers to assist in gaining program acceptance by the community
- Conduct interviews in the home and other locations to identify, inform and assist individuals and families access health, social and other types of services needed to assure positive health outcomes.
- Provides basic case-management services including assessment, reassessment, problem identification, prioritization and resolution, care coordination information and education and crisis intervention.
- Makes referrals for needed services assuring the availability of transportation to needed services and translation services as needed.
- Provides ongoing social and emotional support and assists the patient to learn to utilize the complex health system and other community service systems
- Attends education and in-service training programs.
- Maintains an accurate case record of each CHW participant. Completes required forms and other reports as necessary.
- Participates in Outreach activities to promote the CHW Program and find new Program Participants.
Requirements:
- High School Diploma or equivalent.
- Valid New York State Driver's license, personal car to carry out employment duties and an active automobile insurance policy.
- Basic computer skills. Good writing and verbal skills. Spanish or Creole speaking a plus.
The pay for this position is $21.00 per hour.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
