We are searching for a talented Community Health Worker-Medical
Hours: 40hrs
Shift: Day/Evening shift; 8:30am – 5:00pm
Location: Southcoast Business Center – Fairhaven, MA
Driving on average 3 or more times per week required

Responsibilities:Under the general direction of the Manager of Care Management or designee, in collaboration with a multidisciplinary care team, provide culturally appropriate health education, coaching, support, information and outreach in a variety of settings including community-based, primary care, ambulatory and acute care settings as needed. Identify high risk patients, associated health needs. Facilitate the coordination of community resources to meet the patients plan of care. Assist patients to practice healthy behaviours and access necessary social and medical support services and programs. Conduct home and community visits.
Qualifications:- Equal to completion of 4 years of high school plus additional specialized courses or training is required.
- Bilingual skills preferred.
- Two years experience in community based member outreach.
- Proven ability for independent decision making and problem solving while performing effectively as part of a multi-disciplinary team.
- Solid knowledge of community resources and effective teaching/coaching skills.
- Verifiable good driving record and reliable transportation.
- Community Health Worker certificate preferred.
- Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
