What the Job is: Under the supervision of the Director of Grants and Health Equity, the Community Health Worker (CHW) will be responsible for helping our Hispanic patient population navigate and access community services, understand their health conditions and adopt healthy behaviors - while addressing disparities in health outcomes specific to the population served.
Some of the key things you'll do include assisting your assigned patients with arranging appointments, translation duties, linking them to resources in the community, encouraging them to get necessary screenings for cancer, diabetes and other diseases. You will also help identify and reduce cultural and socio-economic barriers between clients and providers and assist eligible clients to apply for publicly sponsored health care insurance plans, assisting with necessary follow-up, tracking and data collection to assure continuous enrollment.
What experience and education do you need? You must be at least 18 years old and have a high school diploma or equivalent. You must be fluent in Spanish - reading, writing, and speaking. You will also be a driver, so you'll need a valid driver license, verification of personal auto insurance and the ability to become an authorized driver.
A pre-employment drug testing, physical, review of required immunizations including COVID Vaccination and background check required.
