The Community Health Worker is a public health worker who applies their unique experience, language, and culture of the populations they serve to carry out a variety of roles.

Duties & Responsibilities
- The Community Health Worker assess clients and the community to understand the needs, strengths and resources of the individuals and the communities served.
- Act as an advocate and liaison between the enrollee/family and community service agencies.
- The Community Health Worker communicate with culturally diverse patients with varying beliefs, customs, and health disparities.
- CHW act as a contact point, advocate and resource for patients, their family, and their providers, building effective relationships through trust, respect, and communication.
- Excellent knowledge of community resources and referral process or willingness to learn.
- Educate patients or clients to promote healthy behaviors changes.
- The Community Health Worker documents client contact in the electronic health record.
- Help to identify any services that client may be eligible for within the agency and provide warm handoff to appropriate programs as needed.
- Perform home visits to patients or clients when needed.
- Maintain clear professional boundaries regarding personal involvement with patients and clients.
- Other duties as needed.

Required Experience:
Education & Experience
- HS diploma/Ged
- 1-2 years' experience in similar role
- Knowledge of Microsoft Office
- Strong written and verbal communication skills.
- Bilingual is a plus.
