Position Summary
Provide Support and Assistance to Bronx Caregivers. This position requires a bilingual in Spanish leader with a strong program management and administrative background. Provide support groups to caregivers, so they have the opportunity to not only share their own stories, but also, to listen to the stories of other individuals undergoing similar situations. While avoiding isolation and probable depression, group participants can learn from others’ experience to then be able to adjust and deal with their own reality. In a comfortable environment, free of judgment and criticism, caregivers will be able to speak up about their personal situation. Caregivers can be spouses, adult children and young adults. The CHW will be trained to conduct support groups and be supervised by a MSW.
Essential Duties and Responsibilities
- Outreach to 250 Hispanic caregivers in The Bronx and provide support and assistance
- Provide bi-monthly support groups to 120 Hispanic Caregivers in The Bronx and accommodate Caregiver work schedules
- Improve the well-being and quality of life of caregivers of individuals with Alzheimer’s Disease and related Dementias
- Decrease Caregiver stress and increase information and awareness of Alzheimer’s Disease and related Dementias
- Increase awareness of community resources
- Outreach to all R.A.I.N. Programs, social media, places of worship, housing, elected officials community offices, health and social service providers serving and targeting The Bronx
- Create strategies to address program participation
- Ensure compliance with regulatory standards and procedures
- Ensure follow through on program plans, ongoing program development and evaluation and agency systems
- Review logs for program participants
- Support cultural diversity by ensuring that the delivery of quality, equitable and culturally competent client centered services are provided
- Communicate with program officers and appropriate staff
- Establish community linkages with social services and health providers
- Development and implementation of staff/volunteer training
- Represent R.A.I.N. and aging services on external committees and groups
- Competency in State sponsored data base
- Preparation and submission of month Program report
- Adheres to all agency policies, procedures and core values
- Perform all other duties as assigned.

Minimum Qualifications
- High School Diploma, Associates Degree preferred. Have experience as a caregiver for a loved one with Alzheimer’s or related Dementia. Have at least two years of experience working with families, older people, and/or people with dementia
- Substantive experience with senior programs and supervising teams
- Considerable knowledge of Alzheimer’s Disease and related Dementias, gerontology, crises management, social service resources
- Ability to organize, manage and evaluate multiple activities and services
- Ability to develop and maintain effective working relationship s with the community, other staff, volunteers and clients
- Ability to communicate effectively with other team members, community agencies, and clients with diverse opinions, values and cultures
- Ability to track and report on data is essential
- Strong computer skills with experience in Microsoft Word, Outlook and Excel
- Capable of optimizing time and efficiently working with teams
- Bilingual in Spanish required for targeted client population.
