Our client is seeking a Community Health Nurse in Queens, NY, to provide skilled nursing care and assessments to PACE members in homes, hospitals, and PACE centers. You'll play a vital role in coordinating care and working with interdisciplinary teams to ensure quality patient outcomes.
Compensation: $105,000 - $118,000/year
Location: Queens, NY
Schedule: Monday to Friday, 8:30 AM to 5:30 PM

Responsibilities as the Community Health Nurse (RN):
- Patient Care: Conduct assessments and provide direct nursing care, including medication administration and preventative treatments.
- Coordination: Collaborate with interdisciplinary team members and external providers to develop comprehensive care plans.
- Documentation: Maintain accurate, timely, and relevant documentation in compliance with regulatory standards.
- Education: Educate patients, families, and caregivers on medication, safety, and health practices.
- Compliance: Ensure compliance with state and federal regulations, including UAS-NY assessments and eligibility reviews.
- Education: Degree from an NLN accredited school of nursing required (BSN preferred).
- Experience: 1+ year of Clinical Nursing experience in Home Health, Acute Care, or Critical Care (frail or elderly populations preferred).
- Licensure: Current NY State license to practice as a Registered Nurse.
- Skills: Proficient in clinical assessment, care coordination, and communication.
- Other: Valid driver's license, satisfactory health clearance, and up-to-date immunizations required.
