Job Title: Community Care Manager
Location - Remote but Must reside in Palm Beach or St. Lucie County, Florida
Contract - 3 months
Job Summary: As part of the Community-based Care Management program, the Community Care Manager plays a vital role in identifying members/consumers suitable for care coordination and case management services in a community setting. This position involves collaborating with various stakeholders, conducting assessments, and developing care management plans to address the needs of high-risk members. The Community Care Manager will also lead and supervise Community Care Connectors and may serve as a subject matter expert to support program operations.
Key Responsibilities:
- Identify members/consumers for care coordination and case management services in collaboration with the Medical Management department.
- Conduct in-home environmental and physical assessments for high-risk members to identify unmet needs and barriers to care.
- Develop and implement comprehensive care management plans in partnership with the member's medical management team.
- Establish and maintain relationships with community medical and behavioral health teams to facilitate integrated care.
- Direct and supervise Community Care Connectors in providing community-based care, support, and education for chronic disease management.
- Assist the Community Care Program Manager with operational responsibilities and serve as a program subject matter expert as needed.
- Ensure alignment of community-based medical management programs with strategic growth initiatives and the annual operating plan.
- Maintain accurate and timely documentation in electronic medical records and other electronic documentation systems.
- Travel extensively within assigned communities in Florida to meet members in their homes.
Required -
Education: State of Florida Registered Nurse (RN) with at least two years Pediatric experience with medically complex children, or have a Masters Degree in Social Work (MSW) and be a Licensed Clinical Social Worker (LCSW) in the state of Florida with at least one year of related professional experience with medically complex children required.
Qualifications:
- Minimum of three years of experience in care management or a related field, with demonstrated expertise in conducting health assessments and delivering care in clinical and community settings.
- Proven experience in leading teams within a matrixed environment.
- Completion of medical exam/testing as required by Federal and State regulations upon conditional offer of employment and post-employment.
- Proficient in MS Office (Word, Excel, Outlook), internet applications, electronic medical records (EMRs), and electronic documentation programs.
- Valid driver's license and automobile insurance are required.
- Must reside in Palm Beach or St. Lucie County, Florida.
- Ability to conduct monthly face-to-face visits in member homes and travel extensively within assigned communities.
Physical Requirements:
- Ability to perform in-home visits, which may include navigating various environments.
