POSITION OVERVIEW
Licensed Clinical Social Worker (LCSW) at Harbor Health to provide comprehensive behavioral health services and care coordination for our members in collaboration with psychiatry. This role combines clinical expertise with a collaborative approach to support our integrated healthcare model.
POSITION DUTIES & RESPONSIBILITIES
Conduct behavioral health assessments and develop individualized treatment plans
Provide individual, group, and family therapy using evidence-based interventions
Collaborate with primary care providers and specialists to ensure integrated care delivery
Document clinical interactions and maintain accurate electronic health records
Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person.
Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
Provide crisis intervention services when needed
Support development of behavioral health programs and initiatives
DESIRED PROFESSIONAL SKILLS & EXPERIENCE
Master's degree in Social Work and current LCSW license in Texas
Minimum 3 years post-licensure clinical experience
Experience in integrated healthcare settings preferred
Strong knowledge of evidence-based therapeutic modalities
Crisis intervention and risk assessment expertise
Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation).
Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders, when appropriate.
Knowledge of community resources and social service systems
