CNA / Certified Nursing Assistant - Hospice. - Grane Home Health & Hospice.

We are seeking a full-time Hospice Aide/CNA to provide essential personal care services to patients in a home setting, working Monday to Friday from 8 AM to 4:30 PM. This role is vital in supporting patients' needs and enhancing their quality of life.

Responsibilities include:

• Performing personal care tasks and responding to patient requests.
• Communicating effectively with patients and families while maintaining confidentiality.
• Assisting with daily living activities, meal preparation, and medication reminders.
• Monitoring and reporting changes in patient conditions to the Case Manager.
• Maintaining a clean and safe living environment for patients.

Qualifications:

• Completion of state-required training and experience in personal care.
• Ability to work independently and communicate effectively.
• Current CPR certification and a calm demeanor in patient interactions.
