As a Licensed Practical Nurse (LPN) at Chestnut, you will make a difference every day in our housing program providing comprehensive healthcare services to our clients. While upholding the highest standards of professionalism and care, you will contribute to the success of our programs and the well-being of our clients.
Responsibilities
- Coordinate the completion of all required data entry and paperwork, ensuring compliance with funder requirements and deadlines.
- Review medical records to ensure completeness and obtain necessary signatures.
- Provide clinical services including medication administration, monitoring, training, and case management.
- Assess consumers for acute physical issues and coordinate referrals or prescriber reviews.
- Act as a liaison between prescribers, consumers, and healthcare providers to ensure continuity of care.
- Respond to consumer and family inquiries regarding medication refills and concerns.
- Administer and document TB skin tests.
- Provide medical teaching in alignment with nurse practice guidelines.
- Promote the recovery model of treatment and use trauma-informed care principles.
- Participate in team meetings, program development activities, and support other team members as needed.
- Uphold Chestnut's culture of customer service excellence and maintain confidentiality of organizational information.

Qualifications
- High school diploma or equivalent
- Valid practical nurse license in Illinois with at least 5 years of LPN experience
- Basic computer skills, including proficiency in MS Word, Outlook, e-prescribing, and electronic medical record (EMR) systems.
