Job Duties:
Provides advocacy, monitoring and discharge planning forclients with a serious mental illness at the State Hospital andin the Community.
Provides discharge planning and linkage to communityresources for Not Guilty Reason of Insanity (NGRI)Acquittee that are released into the community from StateHospital.
Completes intakes, assessments, individual service plans,updates, discharges and compiles other information insupport of program and licensure requirements.
Completes a
comprehensive assessment of casemanagement needs of NGRI Acquittee.
Develops an ISP and CRP based on the assessmentinformation and in collaboration with the consumer, othertreatment providers, criminal justice staff, and, if appropriate,family, spouse or significant others.
Reviews and updates the CRP at least every 6 months ormore frequently if needs change.
Conducts a face-to-face assessment to determineconsumer’s functional limitations, eligibility for services,progress and need areas at least once a month for theassigned caseload.
More frequent face-to-face contacts willoccur when clinically appropriate or when delineated in theIndividualized Services Plan (ISP) and/ or ConditionalRelease Plan (CRP).
Maintains timely, accurate documentation and data inaccordance with DHS standards, regulatory requirements,and Virginia statutes.
Examples include such items as casenotes, reports to the Forensic Office of the DBHDS andcourt reports. Ensures that all filing of clinical informationand submission of billing information are done accuratelyand within the time frame required.
Coordinates pre-screens through Emergency Services andthe Revocation of NGRI Conditions, as needed.
Coordinates services and service planning with otherproviders within the DHS, outside agencies or providers andthe criminal justice system.
Provides court testimony regarding NGRI consumer’sprogress & compliance with their CRP.
Monitors medication compliance
Participates in case consultations.
Maintains daily Anasazi service logs.
Must be certifi ed as a Qualifi ed Mental Health Professional - Adult(QMHP) through the Virginia Board of Counseling.
Minimum/Special/Additional Requirements:
BEHAVIORAL HEALTH POSITIONS: Requires a four (4) year degree in a humanservices fi eld (such as psychology, behavioral or mental health, specialeducation, social work) and three (3) years’ experience in fi elds providing therequired knowledge, skills, and abilities and associated with such positions as
Mental Health Counselor, Special Education Teacher, or Social Worker OR anycombination of education (above the high school level) and/or experience andtraining equivalent to seven (7) years in fi elds providing the required knowledge,skills, and abilities.
Must be certifi ed as a Qualifi ed Mental Health Professional- Adult (QMHP-A) through the Virginia Board of Counseling.
Must have, or obtain, a current and valid driver’s license.
