Clinical Team Leader - FACT/ACT - Merakey

Earn $72,000-75,000/year, plus mileage reimbursement.

Schedule: Monday-Friday, 8 am-4 pm

Position Details

We are seeking a dedicated and experienced Team Leader to oversee and direct our Assertive Community Treatment (ACT) Team. This role involves managing a self-contained clinical team responsible for providing comprehensive treatment, rehabilitation, and support services to clients with severe and persistent mental health illnesses. The Team Leader will supervise and evaluate a multidisciplinary team, ensure service excellence, and act as a practicing clinician.

Duties and Responsibilities


* Oversee daily clinical operations of the ACT Team, ensuring adequate staff coverage by coordinating work schedules.
* Facilitate daily team meetings and maintain detailed logs of meeting discussions and decisions.
* Manage the client admission process, including scheduling interviews, conducting initial assessments, and coordinating treatment plans with the psychiatrist.
* Provide clinical supervision, including weekly individual and monthly field supervision, while maintaining relationships with staff, clients, and community resources.

Assertive Community Treatment (ACT)

ACT teams are composed of professionals including psychiatrists, nurses, mental health professionals, substance abuse professionals, employment specialists, housing specialists, and a peer specialist, all of whom collaborate to provide complete services including therapy, medication management and resource linkage to adult individuals with serious mental illness (SMI). Services are provided in the individual's home or the community and include at least three home visits a week.

The ideal candidate will possess the following qualifications:


* Masters' Degree in the human services field of Psychiatric Rehabilitation, or Psychology
* Supervisory and program management experience is required
* Professional license eligible as a Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW)

* Strong clinical skills and experience providing treatment to persons with severe and persistent mental illnesses
* Clear understanding of the characteristics and problems of adults with severe and persistent mental illnesses
* Knowledgeable about the Assertive Community Treatment (ACT) model
* Active and unrestricted driver's license is required
