USC Mann School of Pharmacy and Pharmaceutical Sciences is accepting applications for a part-time, (32 hours per week), Clinical Staff Pharmacist with ambulatory care skills to provide comprehensive pharmacy services at Los Angeles Christian Health Centers (LACHC). These services include disease state management and education for the highest-risk patients (those in care transitions, frequent users of acute care resources, patients not meeting treatment goals for common chronic illnesses, etc.), medication therapy management for all patients (at minimum, medication reconciliation), drug information and medical staff education, dispensing and managerial responsibilities at LACHC’s in-house pharmacy, and filling in care gaps related to medication related problems for all patients at the health center. The candidate will also have supervisory responsibilities for pharmacy technicians and clerical staff and ill have teaching responsibilities for USC students and residents rotating through the clinic. Minimum Education: Doctorate in Pharmacy degree from accredited (ACPE) school of pharmacy. Minimum Years of Experience: 1 year. Minimum Field of Expertise: Registered pharmacist in state of CA. Preferred Field of Expertise: PGY1 residency Preferred Experience: Teaching and precepting students in clinic, providing patient care in areas emphasized. The hourly rate range for this position is $70.00 - $75.00.
