Compensation Range:
Anticipated Base compensation rate is in the range of $84,502.00 - $100,055.00 / Year depending on qualifications and experience.Department:
801059 SCN MSSPExpectations:
Under the supervision of the Clinical Supervisor and with minimal supervision required, provides case finding, consultation, and care management services to adults with disabilities and older adults and their families in home and community-based settings in order to preserve health, maintenance of function and continued independence through assessing needs, monitoring quality of care, and providing education, advocacy and support. This position requires flexibility of hours.EDUCATION:
Master's degree from an accredited university in social work, psychology, counseling, rehabilitation, gerontology, sociology or a related field contingent on approval from oversight agency.
EXPERIENCE/TRAINING:
Minimum 2 years experience working with older adults and/or adults with disabilities required. Experience with care management and work in home settings strongly preferred.
SKILLS:
Clinical assessment skills, knowledge of community resources and public benefits, ability to work with a multidisciplinary team and with clients/families in the home setting and by telephone.
Knowledge of aging programs for seniors and/or adults with disabilities.
Computer skills including Microsoft Office or equivalent and ability to learn and utilize specialized software with training and support.
