Performance Improvement Specialist
The Performance Improvement Specialist plays a key role in supporting quality assessment and performance improvement (QAPI) activities for the local hospice branch. This ongoing, data-driven program involves data collection, analysis, and collaborative development of performance improvement initiatives.
Monday-Friday, 8 AM - 5 PM, in-office
Some travel to the Woodlands office required
Job Responsibilities
Conduct ongoing quality assessments through data collection and analysis, collaborating with hospice leadership to identify trends and areas for improvement. Key activities include:
Monitoring Hospice Quality Reporting Program (HQRP) data, including CAHPS and claims-based measures.
Benchmarking HQRP scores against competitors.
Analyzing service comments and CAHPS survey responses.
Maintaining QAPI dashboard and infection control reporting.
Facilitating Root Cause Analysis (RCA) meetings and tracking corrective actions.
Conducting revenue analysis and tracking compliance with training requirements.
Preparing monthly, quarterly, and annual quality reports.
Coordinating performance improvement projects and maintaining documentation.
Acting as liaison to Central Support for QAPI reports and quality initiatives.
Overseeing Performance Improvement Specialist Assistant, if applicable.
Other duties as assigned.
Job Qualifications
Licensed healthcare professional or non-clinician with quality RN or LVN background preferred.
Certified Professional in Health Care Quality preferred.
Knowledge of Medicare/Medicaid regulations and state licensure laws.
Experience in data collection, measurement tools, and quality assurance/improvement.
