About the Job
Requires independent decision making, clinical judgment, and in-depth knowledge of current pharmacotherapy literature. Assess, develop, coordinate, implement, and maintain clinical pharmacy programs and services, including, but not limited to the following activities: * Management and optimization of the Daily Clinical Activities function of the pharmacy staff. * Ensure that clinical pharmacy team provides quality and comprehensive pharmaceutical care across continuum of all patients to ensure safe and effective drug therapy. * Evaluate all drug therapy for appropriate indication, dose, route, dosage form, drug and food interactions, and allergies * Monitor drug regimens for efficacy, adverse drug events (ADEs), and toxicity and intervene as necessary * Identify ways in which drug costs can be minimized through appropriate drug selection * Development and maintenance of clinical competency of the pharmacy staff. * Serve as Secretary of Pharmacy and Therapeutics (P&T) committee * Serve as liaison for P&T committee to the Medical Executive Committee (MEC) * Present items passed through P&T and MEC at appropriate hospital section meetings and other committees upon request of medical affairs. * Ensure appropriate medication use evaluation (MUEs), authorship and presentation of drug monographs, drafting of pharmacy newsletter articles, and conduction of quality assurance activities to improve pharmaceutical care * Develop and implement cost savings initiatives * Development and implementation of drug related policy, procedures, and protocols in order to maximize drug therapy benefits * Identify, report, evaluate, and assist in develop processes to reduce medication errors and prevent ADEs * Implementation of best-practice clinical programs and services. Propose and develop, in collaboration with the Director of Pharmacy (DOP), Medical Affairs, and the Administrative Executive Counsel (AEC), annual goals for the hospital related to clinical services. * These goals should reflect SMART model (Specific, Measurable, Achievable, Relevant, and Time-bound). * Participation in daily delivery of pharmaceutical care, as needed. * Obtain accurate medication histories from patients, families, and patient pharmacies and review medication histories in order to determine which maintenance medications should be continued * Make rounds with other members of the medical team and provide pharmacotherapeutic recommendations * In collaboration with clinical dietitian, evaluate enteral and parenteral nutrition for appropriateness and make recommendations in order to optimize nutrition * Provide pharmacokinetic dosing and monitoring for all medications that require therapeutic drug levels * Provide drug information, intravenous compatibility, and poison information to Physicians, Nurses, and other members of the Cabell Medical Team * Respond to Medical Emergencies as needed (Stroke, AMI, Code Blue, Trauma) * Provide oversight for education of pharmacy students and residents * Ensure staff adherence to standards of the American Society of Health-Systems Pharmacists (ASHP) of a pharmacy preceptor * Serve as Residency Program Director * Educate the medical team members (nurses, MDs, residents, students, etc) on drug therapy, pharmacotherapy issues, drug related policies, procedures, and protocols * Ensure delivery of a variety of mediums utilized to educate hospital stakeholders (oral presentations, Health Stream/online learning, pharmacy department intranet page, and written information). * Ensure the hospital is meeting core performance measures relating to medication use. * Ensure compliance with standards associated with investigational medications and research * Serve as dispensing investigational pharmacist or ensure that sufficient staff is appointed to these roles to ensure safe, effective delivery of these medications according to Investigational Review Board (IRB) approved protocols. * Ensure appropriate order-sets are utilized in the electronic medical record to facilitate the use of investigational medications * Ensure appropriate storage and monitoring of storage conditions of investigational medications according to Food and Drug Administration (FDA) standards. * Participate in professional development activities designed to acquire and maintain skills to support major job responsibilities. * Serves on IRB and collaborates with Marshall University’s Director of Research Development on research initiatives by serving as the acute care liaison for all research medications for receiving, proper storage, and documentation required for all activities. Meets with research teams for onsite reviews and audits. * Communicate regularly and effectively with the Director of Pharmacy other members of the management team on progress and results of major job responsibilities. * B.S Pharm, PharmD or Master's degree related to pharmacy required. * Specialty Residency or equivalent experience (5+yrs hospital as clinical specialist) required. * WV Board of Pharmacy Licensure required. * 5+ Years Position-Related Experience required.
