About the Job
This is a hybrid position.
The Clinical Pharmacist is responsible for providing clinical pharmacy services within the Value Based Care (VBC) Pharmacy Team for the Clinically Integrated Network (CIN). This pharmacist will assist with pharmacy initiatives related to cost reduction, improved patient care and quality outcomes. The Clinical Pharmacist will utilize his/her drug knowledge while working directly with patients, providers, and other members of the health care team to answer medication questions, conduct comprehensive medication reviews, provide education to various stakeholders, review and clarify patient medication regimens and make clinical recommendations to providers. This pharmacist will also help oversee the daily activities of Certified Pharmacy Technicians within the VBC Pharmacy Team and serve as a mentor for pharmacy interns, students and residents when assigned.
- Provides clinical pharmacy services to improve drug usage and therapeutic outcomes including advising physicians and practitioners on issues concerning drug therapy, the inherent toxicity of drugs, drug-drug, drug-food interactions, and side effects, as well as assisting in the prescribing of appropriate doses.
- Provides clinical pharmacy services to patients and/or caregivers to improve medication adherence, medication knowledge and patient outcomes. Services will primary be delivered telephonically with opportunities for virtual and face-to-face encounters as well.
- Provides accurate, adequate, and timely drug information to the professional staff and to patients and their families.
- Appropriately documents pharmacy services provided an Electronic Medical Record (EMR).
- Works collaboratively with the pharmacy team to review and manipulate data in various platforms such as Excel, electronic medical records and/or other population health platforms to recommend, develop and initiate pharmacy initiatives that support value based care contracts.
- Demonstrates pharmaceutical knowledge and competencies related to drug knowledge, PBM proficiency, coordination of care related to medication refills, assistance with medication related problems around cost, transportation, medication delivery and medication questions.
- Participates in departmental program development and Continuous Improvement (CI) activities.
- Performs other related duties as assigned by authorized personnel to provide departmental flexibility to patient care needs including those for emergency situations.
- Carries out rules and regulations pertaining to the professional policies of the department and ensures compliance with State and federal regulations.
- Able to maintain punctual attendance consistent with Nemours policies, regulatory, federal, state and local standards.
- Additional miscellaneous duties and responsibilities, as may be assigned from time to time by management.
Job Requirements
- Bachelor of Pharmacy or Doctor of Pharmacy (PharmD) required.
- Minimum of three (3) years experience required.
- A current active (in good standing) pharmacist license in the State of Delaware is required upon hire.
- An active, in good standing, Pennsylvania pharmacist license is preferred. *Associates who are not licensed in Pennsylvania at the time of hire, must obtain within 12 months after start date; failure to do so may result in suspension pending termination.
- Value-based care and/or pediatric experience preferred.
- Medication Therapy Management Certification preferred.
- Board Certified Pediatric Pharmacy Specialist (BCPPS) preferred.
- Basic Life Support certification preferred.
