Position Summary
Monitoring of teaching service patients, medication and nutrition therapy. Provision of drug information and Pharmacokinetic/nutritional support consults. Educate health care professionals on pharmacotherapeutic management. Serves as a liaison on medication related issues. Precepting and mentoring pharmacy students and pharmacy practice residents. Participates in adverse drug reaction reporting program. Performs other duties as assigned.
Responsibilities
Optimizes pharmacotherapeutic interventions for patients with diabetes within ECU Health system and provides documentation of these interventions.
Assumes an active role in the pharmacotherapeutic management of patients with diabetes which may include credentialing of either/or Certified Diabetes Clinical Education Specialist (CDCES), Board Certified Advanced Diabetes Management (BC-ADM), or Clinical Pharmacist Practitioner (CPP).
Actively participates in Electronic Health Record (EHR) driven initiatives that directly support evidence-based care of patients who have diabetes (ie order set creation/modification, best practice alert creation/evaluation, etc).
Serves as an information resource for the evaluation and implantation of diabetes-related guidelines across the ECU Health system for Nursing, Medical, and Pharmacy staff.
Provides drug information to healthcare providers regarding both insulin and non-insulin diabetes medication therapy.
Ensures patients meet system glycemic goals through appropriate pharmacotherapy interventions.
Educates Medical, Nursing, and Pharmacy staff on appropriate medication use.
Provides education to Pharmacy students and residents through clinical and/or administrative rotation precepting.
Assists in patient education on medication topics in collaboration with Clinical Nurse Specialist staff.
Assists in implementation of Pharmaceutical Care, including in-service training of professional staff.
Serves as liaison between Pharmacy and the units on diabetes-related topics.
Demonstrates the knowledge and skills necessary to provide service appropriate to the age of the patient.
In conjunction with other health care providers, optimizes glycemic therapies to patients.
Ensures seamless transitions of care from inpatient management to ambulatory setting post-discharge.
Collaborates with Diabetes Program Manager and Diabetes Program leadership to ensure metrics of glycemic success.
Reviews all safety events filed surrounding diabetes care across ECU Health system, working in collaboration with Diabetes Program Manager.
Supports and expands safety reporting for patients with diabetes.
Assists in Quality projects and initiatives as determined by Diabetes Program Manager, Executive Leadership, and Medical Director.
Obtains Continuing Education credits as required by the North Carolina Board of Pharmacy.
Maintains active NC pharmacy license and provides proof of renewal prior to December 31 annually.
Maintains active BLS certification and provides proof of renewal bi-annually.
Successfully completes all annual department competency assessments.
Completes the T JC annual requirements and occupational health screening.
Complies with all state and federal laws and regulations governing the practice of pharmacy.
Minimum Requirements
Doctorate in Pharmacy (PharmD) is required.
Must be licensed as a pharmacist by the NC Board of Pharmacy by start date.
3 months to 1 year of specialized pharmacy residency training is preferred.
More than 2 years of experience in practice area may substitute for residency training.
Heart Association Basic Life Support (BLS) Certification
