Job Summary and Qualifications
As a Pharmacy Clinical/Operations Specialist, you will be responsible for participating and leading pharmacy clinical or operational programs. This includes monitoring patient medication regimens and providing appropriate interventions. The Clinical Specialist shall also assist in mentoring pharmacy staff and precepting students/residents and assist in the evaluation and implementation of clinical/operational programs.
What you will do in this role:
- Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles, timely and accurately. Maintains accurate, complete patient medication profiles.
- Delivers clinical, patient problem-based care according to the standards defined within the American Society of Health-System Pharmacist (ASHP) and American College of Clinical Pharmacy (ACCP) Standards
- Acquires and maintains current knowledge in pharmacy practice to promote optimal patient outcomes
- Contributes to the professional development of peers, colleagues, and others by serving as a preceptor for staff, students, residents and other medical professionals
- Decisions and actions on behalf of patients are made as a patient advocate
- Collaborates with the patient, significant other, and healthcare providers in providing patient care
- Considers factors relating to safety, effectiveness, and cost in evaluating, planning and delivering patients care
- Supports and promotes the organization’s safety and risk management programs
- Complies with hospital and department policies and procedures, state/federal laws, regulations promulgated by the government or other third parties, TJC and ASHP standards
- Takes a leadership role within the pharmacy department and hospital, and advances pharmacy practice and patient care services within critical care
- Evaluates drug therapy orders for appropriateness of dosage, route, interval, schedule, and duration throughout patient's hospital course; makes recommendations for change when appropriate
- Takes initiative in identifying ways that drug costs can be minimized through appropriate use of drugs within specialty area
- Provides didactic lectures/presentations to healthcare professionals such as pharmacists, physicians, residents and nurses within area of specialty
- Grows and maintains professional relationship with key physicians within area of specialty
- Conducts planned, written, and systematic evaluation of patient care-related issue or problem; produces program change to improve patient care
- Develops pre-printed orders and/or pathways in conjunction with adult critical care intensivist staff physicians that optimize pharmacotherapy management of adult critical care patients
- Conducts and/or participates actively in the department's medication use evaluation (MUE) program
What qualifications you will need:
- Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy
- Minimum of 3 years’ hospital critical care pharmacy experience required
- OR completion of a PGY2 critical care pharmacy residency
