Clinical Pharmacist - Judge Group

Judge Healthcare is seeking a Clinical Pharmacist for an opening with one of our top clients in the Schaumburg, IL area!

Position Details:

Title: Clinical Pharmacist

Type of Engagement: Contract

Rate: Competitive and Dependent on Experience

Location: HYBRID but must be able to come into office in Schaumburg, IL

Duties:

1. Engage members telephonically and schedule pharmacist consultation appointments with members that participate in any of the clinical programs operationally supported by the Clinical Engagement Services Center.
2. Utilize prescription claims data to generate clinical recommendations according to Medication Therapy Management and Retrospective Drug Utilization Review program protocols providing clinical recommendations pertaining to, but not limited to, gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic substitution, and generic substitution
3. Assess medication regimen for unnecessary medications, ineffective medications, inappropriate doses, and adverse drug reactions.
4. Provide patient education on medication regimen and disease states.

Qualifications:

1. High School Diploma or GED
2. Bachelors Degree in Pharmacy or PharmD
3. Current and unrestricted Pharmacist license in the state of residence
4. Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services (1) Medication Therapy Review Comprehensive and Targeted, 2) Personal Medication List (PML), 3) Medication-Related Action Plan (MAP), 4) Interventions and Referrals, and 5) Documentation of Services and Follow-up)
5. Ability to navigate approved MTM platforms
6. COVID Vaccination required

Preferred Qualifications:

1.PharmD

2. Certified Diabetes Care and Education Specialist

3. Bilingual (with ALTA certification) in Cantonese, Korean, Mandarin, Russian, Spanish, or Vietnamese

4. Additional Pharmacist License in good standing in Alabama, Kentucky, Louisiana, Oregon, or Tennessee

5. 2+ years Pharmacy work experience in retail / community pharmacy and / or PBM, but will consider new graduate experientials

6. Prior call center experience

Hours:

8 hours/day, 40 hours/week
