Spectrum Healthcare Resources has a potential opportunity for a civilian Clinical Pharmacist (Pain) at the Naval Health Clinic Hawaii.

Position Requirements:
- Doctor of Pharmacy (Pharm.D.)
- Graduate from an accredited college by the Accreditation Council on Pharmaceutical Education (ACPE)
- Be a graduate certificate within the preceding year from an accredited pharmacy residency program.Possess a minimum of 12 months of experience within the preceding 18 months as a Clinical Pharmacist performing direct patient care with clinical activities in an inpatient, outpatient, or ambulatory care
- A current, valid, unrestricted license to practice Pharmacy in any state
- BLS
- Provide Medical Therapy Management (MTM) services to eligible adult and pediatric patient populations for medical conditions that may include anticoagulation, diabetes, asthma, hypertension, hypercholesterolemia, and chronic pain.
- Conduct a clinical interview with patient, family, and/or caregiver(s) to identify information necessary to design a pharmacotherapeutic treatment plan
- Participate in a multidisciplinary team with other health care providers providing consultative services pursuant to optimal health care delivery to patients suffering from pain.
- Work with physicians in a multidisciplinary team to provide drug therapy recommendations, perform polypharmacy management, and provide patient education regarding changes in drug therapy.
