Job Description
As a Home Infusion Pharmacist, you will be responsible for providing pharmaceutical care to patients who receive intravenous
medications or therapies in their homes.
Duties and Responsibilities
- Patient Assessment: Conducting thorough assessments of patients to determine their suitability for home infusion
therapy. This may involve reviewing medical records, evaluating lab results, and assessing the patient's overall
health status
- In collaboration with the nursing department, ensure employing a home health nurse prior to the start of care if
appropriate
- Collaborate with healthcare providers, including physicians, nurses, pharmacists, and discharge planners, to
coordinate the transition of patients from hospital or clinic-based care to home infusion therapy
- Collaborate with the billing department, verify patient's insurance coverage and ensure that the prescribed
medication is covered, obtain necessary approvals or explore alternative options if coverage is not available
- Review prescription orders for accuracy, completeness, and appropriateness to ensure that the prescribed
medication is safe and effective for the patient
- Communicate with healthcare professionals to clarify prescription details or obtain additional information.
- Recommend a TPN formula based on clinical experience and practice guidelines
- Ensure that the prescription is properly authorized and meets legal and regulatory requirements
- Maintain and update patient profiles in the pharmacy's system. This includes recording relevant medical history,
allergies, and other pertinent information to ensure safe and appropriate medication use
- Overseeing inventory management, including ordering and maintaining an adequate supply of infusion medications
and related supplies.
- Ensure that pharmaceuticals are accurately compounded and dispensed in accordance with company protocols and
physician orders
- Determine appropriate selection of infusion supplies
- Educate patients and their caregivers on the proper administration, storage, and disposal of medications, as well as
potential side effects, and the importance of adhering to the prescribed treatment care plan
- Be prepared to respond to emergency situations, such as adverse drug reactions, line infections, and equipment
malfunctions, to ensure patient safety
- Document medication errors for Quality Assurance review
- Participate in surveys conducted by authorized inspection agencies
- Report product incidents as required to the appropriate management official
- Manage pharmaceutical recalls in a safe and compliant manner
- Participate in the company’s Performance Improvement Program
- Pursue continuing education appropriate to job responsibilities and license requirements
- Perform other duties as assigned by the supervisor
