Job Description:
We are seeking a dedicated and experienced Pediatric Clinical Pharmacist to join our dynamic healthcare team. The ideal candidate will have a passion for pediatric pharmacy practice and a strong commitment to delivering high-quality pharmaceutical care to pediatric patients.
Responsibilities:
- Collaborate with healthcare providers to optimize medication therapy for pediatric patients across various settings, including inpatient, outpatient, and specialty clinics.
- Conduct comprehensive medication reviews and provide evidence-based recommendations to ensure safe and effective medication use in pediatric patients.
- Participate in pediatric multidisciplinary rounds, providing medication-related expertise and contributing to patient care discussions.
- Provide drug information and education to healthcare providers, patients, and caregivers regarding pediatric medication therapy, dosing, administration, and adverse effects.
- Develop and implement pediatric-specific medication protocols, guidelines, and clinical pathways to standardize care and improve patient outcomes.
- Participate in medication safety initiatives, adverse drug event monitoring, and quality improvement projects related to pediatric medication use.
- Serve as a resource for pediatric pharmacotherapy knowledge, staying current with best practices, guidelines, and emerging therapies in pediatric pharmacotherapy.
Qualifications:
- Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy program.
- Completion of a PGY2 residency in pediatric pharmacy or equivalent experience is preferred.
- Active pharmacy license in Texas
- Board certification in pediatric pharmacy (BCPPS) is desirable.
- Demonstrated experience in pediatric pharmacy practice, preferably in an acute care or ambulatory care setting.
- Strong clinical pharmacy skills, including pharmacotherapy knowledge, medication management, and patient counseling.
- Excellent communication and interpersonal skills, with the ability to collaborate effectively within a multidisciplinary team.
- Commitment to continuous learning and professional development in pediatric pharmacotherapy.
- Ability to prioritize workload, manage multiple tasks simultaneously, and work efficiently in a fast-paced environment.
