We are seeking a highly skilled and dedicated Permanent Clinical Pharmacist to join our dynamic healthcare team. The successful candidate will be responsible for managing all aspects of medication therapy in our fast-paced medical setting. This role involves a high level of interaction with patients, physicians, and other healthcare professionals, requiring excellent communication skills and a strong dedication to patient care. The ideal candidate will have a profound understanding of pharmaceutical therapy, a keen eye for detail, and the ability to work effectively in a team-oriented setting.
Responsibilities:
1. Evaluate and interpret physician orders for appropriateness and accuracy, ensuring all prescriptions are dispensed correctly.
2. Provide patient counseling and medication therapy management services.
3. Monitor patient drug therapies, including over-the-counter (OTC) medications and dietary supplements, for drug interactions, contraindications, and adverse reactions.
4. Develop, implement, and assess pharmaceutical care plans for patients.
5. Collaborate with healthcare team members to optimize patient care, participating in rounds and making recommendations for drug therapy changes as necessary.
6. Participate in the development and delivery of educational programs for the healthcare team, patients, and their families.
7. Maintain up-to-date knowledge of new drugs, medical device regulations, therapies, and treatment protocols.
8. Ensure compliance with all federal, state, and local laws and regulations related to the dispensing of drugs and controlled substances, and the maintenance of required records.
9. Use the Epic system for maintaining electronic health records and managing pharmacy operations.
Qualifications:
1. Doctor of Pharmacy degree (PharmD) from an accredited institution.
2. Current, unrestricted Pharmacist license in good standing.
3. Minimum of 2 years of experience as a Clinical Pharmacist in a hospital or similar medical setting.
4. Proficiency in the use of Epic system for managing pharmacy operations.
5. Experience with 457b plans is preferred.
6. Excellent verbal and written communication skills.
7. Strong analytical skills and ability to make decisions based on clinical judgment.
8. Ability to work effectively in a fast-paced, high-stress environment.
9. Demonstrated commitment to providing outstanding patient care.
10. Knowledge of current pharmaceutical practices and clinical pharmacy standards.
11. Proficiency in Microsoft Office Suite, particularly Excel and Word.
12. Ability to work flexible hours, including nights, weekends, and holidays as needed.
