We are seeking an exceptional pharmacists to lead our Chronic Care Management (CCM), Transition Care Management (TCM), and Remote Patient Monitoring (RPM) services. This full clinical service program will operate through a Collaborative Practice Agreement with independent medical facilities.
As a Fellow , you will establish and maintain professional relationships with all team members within the medical practice. Responsibilities may vary depending on the unique needs of each independent practice, offering you a diverse and stimulating workload. Must live in a commutable distance to the posted job location – this role is performed in provider clinics and may require regional travel to a group of clinics.
In this role, you will have the opportunity to:
- Build and work directly with your patient panel to drive clinical outcomes and results.
- Spearhead patient enrollment, device management, and data interpretation for RPM.
- Collaborate on medication management with supervising providers.
- Participate and lead topic discussions for current staff and residents.
- Precept PGY-1 Students and participate in In-Clinic Rx Fellowship Program.
- Develop individualize care plans based on specific patient needs.
- Provide recommendations or implement therapy changes, when necessary.
Must Haves:
- A current, valid license to practice pharmacy in North Carolina.
- Current CPR certification.
- Certification for Immunization.
