TheClinical PharmacistHome Medication Reconciliation/Transition of Carewould perform an accurate and comprehensive medication reconciliation process, implement and operate an evidence-based medication reconciliation system that optimizes available resources, and promote medication reconciliation as a focus of performance-improvement activities. The Clinical PharmacistTOC / Medical History serves as a patient advocate when planning and implementing pharmacy care by considering each patient's disease state and background.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The responsibilities include but are not limited to the following:
- Responsible for obtaining comprehensive preadmission medication history from multiple available courses and patient interviews.
- Documenting the compiled medication list, thoroughly analyzing possible drug interactions, evaluating patient compliance, and any medication adherence barriers.
- Summarizing all gathered info in clinical notes and being able to express yourself clearly in professional writing
- Being a productive and efficient team member to meet daily goals and expectations
- Overseeing PGY1 pharmacy residents’ daily medication reconciliation process and serving as a clinical preceptor and mentor. To effectively communicate with the residents, provide constructive feedback, and process improvement suggestions.

- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
Qualifications
Education
- Bachelor's Degree in Pharmacy or Pharm D degree. (Required)
- BCPS Certification Preferred
- Completion of Accreddited Residency Preferred
Work Experience
- 3-5 years of experience preferred
Knowledge, Skills, & Abilities
- Effective communication skills are essential (Required proficiency)
- Computer and office equipment, automated compounding systems. (Required proficiency)
Licenses and Certifications
- Registered Pharmacist (R.Ph) in the State of Ohio (Required)
- Licensed practitioner of health sciences profession in the State of Ohio (Required)
