About the Job
The Clinical Services Liaison – RN, is responsible for:
- Positive and effective representation of Catholic Health Home Infusion Program and introducing the full scope of services offered to physicians, nursing agencies and other referral sources.
- Ensuring the ongoing updates and education for referral sources.
- Responsible for the successful clinical coordination of care from the referral source into the home setting on behalf of The Program.
- Provides and utilizes clinical expertise and skills to assist referral sources &/or patients with:
- The identification of appropriate candidates for home infusion &/or home health care
- The identification of the patients’ and/or caregivers’ ability to provide care.
- Patient education and training regarding therapy and supplies, as well as, clarifying expectations of care in the home.
- Medication connection at the hospital / facility to expedite the discharge into the home setting, as appropriate.
Responsibilities:
- Develops, maintains, and grows core therapy and service line admissions to The Program through positive referral source relationships and provides positive and responsive customer service with assigned target accounts.
- Recognizes opportunities for expanding business opportunities with current referral sources / business associates and involves either the Director of Sales & Marketing or the General Manager. (Based on reporting structure).
- Demonstrates thorough knowledge of core therapy referrals and qualifying criteria by payor source, such as: TPN, Enteral Nutrition and Inotropic Agents for Medicare.
- Evaluates current and future clinical and service needs for existing and new accounts.
- As appropriate &/or required, coordinates &/or conducts in-services and continuing education programs for referral sources and their support staff.
- Collaborates with referral sources (i.e. hospital case managers, home health agencies, physician offices) and The Center staff to facilitate the coordination of care for new patient referrals to The Center.
- Coordinates and communicates all discharge planning activities for new patient referrals with The Center personnel and referral sources, including, but not limited to:
- Intake
- Clinical Operations: Pharmacy &/or Nursing
- Warehouse Operations, if required
- Reimbursement, if required
- Appropriately documents the coordination of care activity including, but not limited to:
- Home infusion &/or nursing services utilized.
- All communication with patients, referral sources, physicians, and Center personnel.
- All patient &/or caregiver education & training.
- Documentation must be legible, accurate and complete and complies with company policies & procedures.
- Maintains up-to-date referral source records, as well as any other requested records in accordance with The Center’s instructions &/or business goals.
- Assist with activities to achieve favorable net revenue and gross profit goals for The Center.
- Represent The Center in a professional and positive manner.
- Attend and participate in appropriate office meetings, sales meetings, training programs and regional and national trade shows and conventions as directed by Director of Sales & Marketing or the General Manager, (Based on reporting structure).
- Coordination and cross-training of departmental work duties to achieve maximum productivity and efficiency.
- Stay abreast of developments in field of infusion therapy.
- Comply with all company policies and directives for the fulfillment of the company objectives.
- Communicates any problems incurred directly to the Home Infusion Pharmacy Manger
- Perform other duties as directed by manager.
Minimum Requirements:
- RN (Registered Nurse) – Diploma, Associates or Bachelor’s degree from an accredited college or school of nursing.
- Strongly Preferred: 3 years of IV related experience
- 2 years of Home health care experience 1 year of Case Management experience Sales and Customer Service experience
- Current licensure as a NYS Registered Nurse
- Must possess a valid and current driver’s license and auto insurance.
- Must have and maintain current CPR certification.
- CRNI preferred
- Must maintain a professional image and abide by company dress code.
- Must have excellent customer service skills (both verbal and written) and have effective interpersonal skills that would allow for positive interactions with varied personality types.
- Proficient computer skills
Physical Requirements
- Required to drive. Regularly works in outside weather conditions when traveling.
- Frequently required to use hands to demonstrate medication administration, use of equipment and administer medications, such as connection of medications at the hospital to facilitate discharge.
- Frequently required to stand, walk, sit, and reach with hands and arms.
- Frequently required to lift and/or move up to 30 pounds.

The pay for this position ranges from $77,000 - 84,000 per year.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
