Key Responsibilities:
Clinical Oversight:
- Receive case referrals and assess patient needs through various means, including home visits and record reviews.
- Assign appropriate clinical personnel to cases and confer with the Medical Director regarding eligibility for services.
- Review patient diagnoses, prognosis, medications, and clinical course to establish therapeutic goals and develop comprehensive care plans.
- Conduct quarterly record reviews and provide guidance to clinicians to ensure the delivery of quality home care services.
Leadership and Supervision:
- Assist in the screening and interviewing process of new employees and provide recommendations for employment.
- Support the orientation of new employees and facilitate their integration into the team.
- Collaborate with the Clinical Director/Director of Patient Care Services in planning, implementing, and evaluating educational programs and organization policies.
Community Engagement:
- Assess community health needs and contribute to the development of programs to meet those needs.
- Participate in public relation and community activities to promote the organization's role in the hospice and healthcare delivery system.
- Foster a customer service-oriented approach among hospice personnel to ensure exceptional patient care.
Quality Improvement:
- Actively participate in quality assessment performance improvement teams and activities to enhance service delivery.
- Perform other duties and activities as delegated by the Clinical Director/Director of Patient Care Services.
Qualifications:
- Current licensure as a Registered Nurse in California.
- Graduation from a National League of Nursing accredited school of nursing, with a preference for a BSN degree.
- Minimum of one year of recent professional nursing experience, with previous experience in hospice/home care and at least two years of management or supervisory experience.
- Strong leadership and communication skills with the ability to work effectively in an interdisciplinary setting.
- Proficiency in observation, nursing judgment, and pain/symptom management principles.
- Valid driver's license, CPR certification, and reliable transportation.
- Must be able to reliably commute to San Marcos.
