1 Job Summary: The Home Infusion Dietitian is a credentialed home
nutrition support specialist or expert with the ability to direct
initiatives for quality and process improvement and protocol
development. Under minimal supervision, the Home Infusion Dietitian
provides clinical support and medical nutrition therapy to home enteral
and parenteral nutrition patients. The Home Infusion Dietitian is
responsible for assessing insurance reimbursement prior to accepting
patients for home infusion services, while also assessing, planning,
implementing, monitoring, evaluating, discontinuing and documenting the
nutrition plan of care in collaboration with the interdisciplinary team.
This individual participates in quality improvement and strategic
efforts to improve nutrition and home care services provided by
Froedtert Home Infusion. EXPERIENCE DESCRIPTION: A minimum of three
years of experience in direct patient care is required, two of which
includes experience with enteral and parenteral nutrition. Must
currently hold a CNSC credential or obtain within 6 months of hire.
Pediatric nutrition experience desirable Nutrition support position for
Froedtert Home Infusion, remote and on-site work Two days per week &
occasional on-call duties EDUCATION DESCRIPTION: Bachelors or Masters
degree from an accredited Clinical Dietetics and Nutrition program is
required. Completion of an ACEND approved internship or coordinated
undergraduate program is required. LICENSURE DESCRIPTION: Current
Registered Dietitian Nutritionist (RDN) is required. Current RDN must
also be a State of Wisconsin Certified Dietitian or show proof of
application for certification within 60 days of hire or transfer.
Current RDN must hold and maintain Certified Nutrition Support Clinician
