Position Overview: The License Practical Nurse (LPN) Clinical Coordinator is responsible for supporting Open Systems Healthcare’s (OSH) clinical operations under the direction of the Director of Nursing, Administrator. This role provides clinical administrative support associated with case management, clinical document management, clinical staffing, and incident/accident reporting and investigations. In collaboration with the clinical team, the Clinical Coordinator is responsible for organizing data, coordinating multiple concurrent activities, and interacting with individuals and groups across all levels of the organization.
Essential Job Functions:
- Coordinate client care by assisting with case orientation, care insight review, and care coordination efforts.
- Responsible for MD and Interim Order management by tracking order completion, ensuring signatures are captured, and documents uploaded appropriately following Agency policy.
- Coordinate PDRN and RN scheduling for post-fall and resumption of care visits.
- Coordinate with clients and external care team on post-hospitalization discharge planning.
- Maintain accurate clinical records by ensuring proper file storage within the branch office and electronic filing systems.
- Works in conjunction with Director of Nursing, Administrator and the Quality and Compliance Manager to ensure client records are audited in compliance with Agency policy.
- Provides clinical administrative support and assistance pertaining to the intake process under the direction of the Director of Nursing, Administrator.
- Manages DC Care Connect and Comagine to ensure accurate documentation and timely document upload.
- Maintains and adheres to Agency procedure and client care policy manuals.
- Follows policies and procedures that are consistent with recommended Centers for Disease Control (CDC) and Occupational Safety and Health Agency (OSHA) guidelines for safety, universal precautions and infection control procedures.
- Follows ongoing quality assurance program according to the District of Columbia Regulation and Agency policy.
- Ensures client confidentiality is maintained according to HIPAA law.
- Provides client support as a community liaison to educate clients, families, and direct support staff to ensure seamless recertification and quality care.
- Ensures accuracy, completeness, and timeliness of documentation in accordance with Agency policies and procedures, regulatory requirements, and industry standards.
- Ensures that the professional standards of community nursing practice are followed.
- Adheres to Agency policies and procedures.
- Adheres to educational programs regarding appropriate documentation practices.
- Promotes personal safety and a safe environment for clients and direct care staff.
- Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures and regulatory requirements.
- Follows the Tuberculosis screening program and Hepatitis B program, if applicable.
- Follows Federal, State and local laws and regulations pertaining to home care services.
- Performs other duties as assigned.
Requirements
Required Education and Experience:
- Graduate of an accredited school of professional nursing.
- Homecare experience preferred.
Required Credentials:
- Current and valid DC LPN license.
- Evidence of Tuberculosis screening that meets CDC guidelines.
- Current and valid CPR certification.
- Current State mandated CEU’s.
- Passed OSH competency exam with a score of 80% or greater.

Work Environment and Physical Demands:
- Ability to perform tasks involving physical activity, which may include heavy moving, bending, lifting, and extensive self-positioning. Continual or almost continual standing may be required.
- The employee is occasionally exposed to a variety of conditions at consumer sites.
