Job Summary:
The Clinical Coordinator will play a key role in overseeing patient care and managing the clinical team to ensure quality, compassionate, and effective care is provided to our residents. The Clinical Coordinator will work closely with the nursing staff, residents, and their families to promote a safe, caring, and positive environment that meets the physical, mental, and emotional needs of residents in our skilled nursing facility.
Key Responsibilities:
- Coordinate Resident Care: Oversee the day-to-day clinical operations and ensure residents receive high-quality care that meets regulatory standards. Conduct assessments and create care plans in partnership with the interdisciplinary team.
- Supervise Clinical Staff: Lead, mentor, and support nursing staff, including RNs, LPNs, and CNAs. Provide training, development, and performance management to maintain a competent and compassionate team.
- Quality Assurance: Monitor and ensure compliance with healthcare regulations, including state and federal standards. Participate in quality assurance programs, conduct audits, and make recommendations to enhance care quality and efficiency.
- Collaboration: Work closely with other departments to coordinate care plans and interventions that support resident well-being.
- Resident and Family Engagement: Communicate regularly with residents and their families to update them on care plans and address any concerns or needs, ensuring a resident-centered approach to care.
- Documentation and Reporting: Ensure accurate and timely documentation of care activities, assessments, and resident progress. Provide regular reports to the management team on clinical performance, staffing needs, and resident outcomes.
Qualifications:
- Education: Bachelor’s degree in Nursing (BSN) preferred; Associate’s degree in Nursing (ADN) with relevant experience will be considered.
- Licensure: Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Nebraska.
- Experience: Minimum of 2 years of experience in long-term care or skilled nursing, with at least 1 year in a leadership or supervisory role.
- Skills: Strong organizational and communication skills, ability to handle multiple priorities, and a commitment to resident-centered care.
