Location: New York, NY
Compensation: $64,000 - $68,000/year
Schedule: 9:00 AM to 5:00 PM, Monday to Friday (Off-Hours Available)

Responsibilities as the Clinical Coordinator:
- Team Supervision: Supervise a team of Case Managers, providing regular, structured supervision meetings.
- Case Management Strategies: Promote effective case management strategies following Housing First, Harm Reduction, CPI, Motivational Interviewing, and trauma-informed care models.
- Service Plans: Develop and implement individual service plans; ensure comprehensive and timely documentation of client contact and progress.
- Client Assessment: Interview and assess potential clients.
- Clinical Leadership: Provide clinical direction and leadership, troubleshoot client engagement issues, and strategize solutions for challenging behaviors.
- Housing Placement: Foster a culture emphasizing housing placement as a success measure; ensure client readiness for housing.
- Care Coordination: Coordinate care delivery with multiple service providers, particularly outreach teams, to ensure client success in transitional housing.
- Conflict Management: Support front-line staff in managing difficult situations and de-escalating conflicts.
- Case Conferences: Supervise and conduct case conferences.
- Education: NYS LMSW required.
- Experience: 3+ years with relevant populations (supervisory experience preferred).
- Skills: Knowledge of Housing First, Motivational Interviewing, Harm Reduction, and health systems.
- Communication: Strong verbal and written communication skills.
- Technical: Proficient in Microsoft Office and familiar with databases.
- Language: Bilingual preferred.
