The Clinical Case Manager is responsible for performing a wide variety of client care activities, as it relates to case management services, treatment planning, continuing care planning, and discharge planning. The Clinical Case Manager will collaborate with Treatment Team members to formulate individualized treatment plans and assist in coordinating discharge planning for each patient. The Clinical Case Manager will also have the responsibility for maintaining communication with the referral source and other services provided.
DUTIES, RESPONSIBILITIES, & ESSENTIAL JOB FUNCTIONS
- Case Management
- Provides case management sessions to clients at the necessary frequency per Clinical Director.
- Document the client’s progress accurately and in a timely manner.
- Present psychosocial and diagnostic information to treatment team in a clear, concise manner.
- Treatment Planning
- Review initial treatment plan and any case management issues during first session with client and ensure the client and team responsibilities are understood and in progress.
- As needed, develop clearly stated treatment goals and objectives that are SMART (specific, measurable, attainable, realistic, and timely).
- As needed, develop interventions that will accomplish goals and objectives and state the planned frequency of interventions as it pertains to continuing care.
- Continue treatment planning process throughout patient’s treatment stay (i.e., medical, legal, and employment issues, problematic behaviors, etc.) and coordinate with treatment team to complete plan.
- Continuing Care Planning, Discharge Planning, and Referrals
- Review assessments from referring facilities and identify potential case management issues that require further attention and remediation (i.e., housing, support system, recovery resources, etc.).
- Assist client with internal and external resources (i.e., community agencies, local support groups, etc.) as needed on continuing care plan.
- Assist client in managing outside stressors (legal, financial, vocational, etc.) according to the best interests of the client’s progress in recovery.
- Contact referral source(s) and other key individuals from client’s care team to gather additional information, as necessary.
- If applicable, maintain regular contact and open communication with families, and provide them with additional resources.
- Maintain open communication lines with facility staff, attending clinicians and physicians, clients and families.
OTHER FUNCTIONS AND RESPONSIBILITIES
- Assist in clinical interventions and staffing of clients, if needed.
- Performs other duties as assigned.
QUALIFICATIONS
- High School Diploma required.
- Associate degree or higher, preferred in either social work, counseling, human services, business administration, or closely related field.
- One year of experience in a substance use and/or mental health treatment environment required.
- Experience in start-up environments is preferred.
- Experience working with state licensing requirements and accreditation standards preferred.
- Demonstrates critical thinking, sound reasoning and judgement.
- Effective communication skills and processes.
- Fosters a healthy and positive team-approach with staff.
- Demonstrated computer proficiency.
COMPETENCIES -- KNOWLEDGE, SKILLS, and EXPERIENCE
- Ability to identify client needs that cannot be met by the counselor or agency and assists clients to utilize the support systems and community resources available.
- Demonstrate knowledge of accepted principles of client record management and document appropriately
- Ability to establish rapport, address and assist client in building and achieving measurable treatment goals and objectives
- Ability to provide education and information to individuals and groups, through formal and/or informal processes, concerning their substance abuse and/or mental health issues.
- Demonstrate appropriate privacy practices and adherence to HIPAA and confidentiality policies when responding to inquiries for client information or copies of information.
- Demonstrated adherence to acceptable ethical and behavioral standards of conduct.
- Demonstrates the ability to prioritize tasks and responsibilities and complete duties in the allotted time.
- Experience working with EMRs: Sunwave Health preferred.
PHYSICAL DEMANDS
- Ability to lift up to 25 pounds.
- Ability to walk up and down stairs during emergency drills or situations.
