POSITION TITLE: Clinical Case Manager II – Supported Employment

POSITION DUTIES:  

Provide culturally and linguistically competent supported employment services to ensure attainment of project goals and objectives for Mandarin-speaking and other members of the Asian American and Pacific Islander community who are job seekers with a disability, in particular with chronic mental illness. Work with clients with a disability, mostly mental illness, to develop and implement strengths-based supported employment vocational/training service plan. Conduct comprehensive assessments addressing broad range of life domains and develop individualized service plan. Develop, implement, monitor, and modify a written personalized service plan in collaboration with clients to provide individual benefit counseling, job readiness assessment, vocational counseling, job coaching, assess progress in clients’ service plans, and assist in job search activities. Deliver client services based on the employment plan. Assist clients and provide advocacy and coordination in accessing support services, training programs, and resources. Provide long term follow-up case management. Prepare concise, complete, and accurate program documentation. Maintain complete and accurate case files and records. Perform job development and program outreach functions in the community. Provide training to and consultation for staff regarding supported employment service to individuals with disability. Participate in training, staff and team meetings, consultations, and conferences. Coordinate and cooperate with other agencies and organizations to facilitate implementation of service plans. 40% telecommuting within MSA.

MINIMUM REQUIREMENTS:

Master’s degree in Social Work, Social/Behavioral Science, Marriage and Family Therapy, or related field, or the foreign degree equivalent. Mandarin language fluency required.

WA state Mental Health Counselor License, Mental Health Counselor Associate License, Social Worker License, or Social Worker Associate License required.

SALARY: $64,605/year to $98,573.23/year
