We are looking to build our Enhanced Clinical Community Support Program and are seeking individuals to fill the new position of Clinical Case Manager. Kaleidoscope's Enhanced Clinical Community Support (ECCS) program provides immediate, short-term services to stabilize youth in care of DCFS in their home after inpatient psychiatric hospitalization. The Clinical Case Manager will work alongside the ECCS Community Clinician to ensure that the young person receives the intensity of services they need in a community-based, family setting. The Clinical Case Manager is responsible for providing a variety of community and home-based case management services to assigned children, youth, and families within the ECCS Program, with a primary focus on mental health case management services.

Job Duties and Responsibilities:
- Demonstrates agency clinical philosophy in all aspects of program activities.
- Works collaboratively with the ECCS Community Clinician to understand youth and family needs upon discharge from the hospital.
- Participates in clinically focused team meetings facilitated by the ECCS Community Clinician to understand service needs.
- Ensures psychotropic medication is obtained for the youth by communicating with DCFS Consent Unit and pharmacy, and provide ongoing support and advocacy when barriers arise.
- Completes referrals to community based mental health programs.
- Find and provide linkage to other community-based programs as needed, such as mentoring, after school programming, etc.
- Attends appointments with families, as needed, to understand the care needs and be able to advocate and provide the appropriate support to youth and their caregivers.
- Completes documentation for youth assigned to caseload, such as progress notes and quarterly reports.
- Maintains an awareness of community resources appropriate to child, youth and family needs, and makes proper use of these resources through referral and follow-up.
- Outreaches to new community organizations to provide linkage to services for families in the ECCS Program.
- Participates in caregiver support group, facilitated by the ECCS Team.
- Utilizes trauma informed interventions such as ARC, SPACE, or other evidence-based models that meets the various clinical needs of children, youth, families, and collaterals on assigned caseload.
- Consults regularly with supervisor, and other service entities to ensure mental health, permanency, safety and wellbeing needs of youth and family are being met.
- Participates in conferences, court hearings, team meetings, staffings, and other meetings in the community as required as an advocate for children, youth, and families, or as a representative of the agency.
- Demonstrates a working knowledge of ECCS program requirements, COA accreditation standards and related DCFS policy and procedures.
- Completes other duties as assigned by the ECCS Supervisor.
General Requirements
Minimum Degree Required: Master's Degree in social work or social service related degree.
Licensure: LCSW or LCPC licensure preferred, not required
Information Technology Skills Required: Microsoft Office Proficiency
Schedule: This job requires a flexible schedule and the ability to work evenings (example: 10am-6pm, 11am-7pm)
Other requirements:
- Excellent interpersonal, oral, and written communication and time management skills.
- Must be able to use Word, Outlook, and video conferencing services.
- Full time use of minimally insured operating automobile and valid driver's license is required.
