Accountability Objective:
This position is a highly responsible position that is responsible and accountable to the Director of Nursing and/or the Assistant Director of Nursing in providing quality of care and meeting federal and state regulations. This position is responsible for directing the staff and in implementing a nursing care program that assures safe and competent care to all residents. This position promotes maximum deliverance of quality of care and quality of life by identifying and resolving problems, issues, and providing appropriate training to the staff. This position is delegated the administrative authority, responsibility, and accountability necessary for carrying out the assigned duties.
Reporting Relationships:
This position reports to the Assistant Director of Nursing / Director of Nursing
Qualifications:
Must be a graduate of an accredited school of nursing and possess a valid RN or LPN license in the state the facility is located.
Preferably an RN.
RN's must have at least one (1) years experience as a supervisor in a long-term care facility or hospital.
LPN's must have at least two (2) years experience as a charge nurse in a long-term care facility. Must possess knowledge in the Resident Assessment Instrument.
Must possess knowledge in Medicare and Medicaid regulations and coverage criteria for long term care.

Job Duties:
- Assist in developing and implementing methods for coordinating nursing service with other resident services.
- Ensure that all nursing personnel on their unit are following their respective job descriptions.
- Participate in facility surveys and is on-site during surveys made by authorized government agencies and performs other duties (during surveys) as requested by the Administrator or director of Nursing.
- Participate in developing, implementing, and periodically updating the resident's care plan.
- Performs administrative duty such as, completing medical forms, reports, evaluation, studies, charting, as assigned by the Assistant Director or Director of Nursing.
- Rotates weekends with the other Resident Care Managers and Assistant Director of Nursing.
- Participate and completes detailed rounds on their unit to ensure compliance with all state and federal regulation and implementation of the residents care plan.
- Coordinates the unit in maintaining accurate bathing, feeding schedules and team
assignments.
- Attends the residents' care plan conference.
- Assist with investigation of complaints and allegations of abuse as directed by the assistant director or director of nursing service.
- Monitors the residents' treatments and medications to ensure residents are receiving proper care.
- Reviews nurses' notes to ensure proper documentation is maintained relating to residents' treatments, medications and conditions.
- Participate as directed by the Director of Nursing in evaluation of nursing employees under his/her direct supervision.
- Meets with staff on a timely basis including staff on other shifts to ensure communication, continuity of care and resolution of problems.
- Attend department head meetings and nurse management meetings as directed by Administrator and/or Director of nursing service.
- Attends family and resident council to develop relationships; ensure resident care is individualized and assists in resolving issues.
- Perform treatments in absence of a treatment nurse.
- Reports resident changes, complaints or staff problems to Director of Nursing/ Assistant Director of Nursing at least every twenty-four (24) hours.
- Calls physician regarding care.
- Reports' labs and any pertinent data to the Physician for possible treatment orders.
- Monitors residents' weight and takes appropriate action.
- Documents in the medical record a summary once every three (3) months addressing the resident's problems, strengths and progress in relation to the plan for care.
- Assist in obtaining needed information for quality assurance and/or infection control reports.
- Participates in the MDS assessment as directed by the assessment coordinator.
- Makes assignments or reassignments based on resident need and availability of staff.
- Documents a daily clinical note for residents receiving skilled care
( Medicare) and residents with significant change of status, including but not limited to, falls, antibiotics, fever, infections, injuries and residents receiving IV fluids, etc. until the condition is resolved.

Must be able to cope with mental and emotional stress of the position
Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, personnel and support agencies.
Must be able to work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
May be required to lift residents, medical equipment, supplies, and minimum weight of 100 lbs. with assistance.
Must be able to attend out of town meetings.
Extensive hours may be required
