WHAT YOU'LL DO:
- The Clinic Care Coordinator is a Registered Nurse who functions as a vital member of the primary healthcare team.
- This nurse is responsible for care coordination services in managing the health of the Medicare populations within the district who are attributed to MCHD through the Texas Panhandle Clinical Partners Accountable Care Organization.
- This includes direct and indirect patient contact. Coordination of Care will emphasize controlling health care costs through implementation of evidence-based interventions focused on improving the client’s overall health across the continuum of care.
- The Care Coordinator will work intricately within the software provided by the TPCP ACO, including AaNeel or replacement of, to enter health information and address Care Gaps.
- Key responsibilities include but are not limited to: coordinating and performing Annual Wellness Visits at various locations throughout MCHD including patient screenings and assessments in accordance with CMS guidelines, ensuring appropriate documentation and billing codes are submitted according to Medicare guidelines on all AWVs, assisting with closing of care gaps, distributing Medicare Risk Adjustment reports to providers and follow up of completion, and monitoring of utilization of resources for attributed members.
- The Care Coordinator nurse will work in collaboration with the primary care team and will delegate according to guidelines provided by the Texas Board of Nursing.
- This nurse is highly engaged in outreach efforts, including communication with physicians, clinic staff, and community members. Interdisciplinary collaboration with various healthcare professionals is required.
- This role will include cross training within the population health department as needed.
EDUCATIONAL/EXPERIENCE REQUIREMENTS: Registered Nurse with minimum of 3 years of RN experience - at least one of these years occurring in the last 12 months. BSN preferred.
