What You’ll Do
As a Service Coordinator, you will oversee developmental disability services for individuals enrolled in Empower programs such as Recreation & Wellness and Transition Programs. Your key responsibilities include:
- Program Supervision: Supervise and ensure the effective delivery of developmental disabilities services at assigned locations.
- Compliance & Best Practices: Monitor services to ensure adherence to state, federal, and agency standards, focusing on personal outcomes for individuals and their families.
- Enrollment & Documentation: Complete necessary enrollment documents for Electronic Health Records (EHR) and Provider Direct, and coordinate with the Billing Specialist.
- Consent & Authorization Management: Support Empower management by ensuring that all consents and authorizations are in place before services begin, and that services remain within authorized limits.
- Referral & Person-Centered Planning: Oversee the referral process, ensuring the development of person-centered plans, short-range goals, consents, and necessary documentation (e.g., NCSNAP).
- Progress Reporting: Regularly report progress on individual goals to program supervisors, direct staff, and families, using available resources to meet the needs of program participants.
- Collaboration: Work closely with Managed Care Organizations (MCOs) and other service entities to ensure the delivery of services aligned with the desires and needs of participants.

What We’re Looking For
To be successful in this role, you should meet the following qualifications:
- Education: Bachelor’s degree from an accredited college or university.
- Experience: Minimum of 2 years of experience working with individuals with intellectual and/or developmental disabilities. Must meet the qualifications for a Qualified Professional.
- Licenses & Certifications: Valid, unrestricted state driver’s license and personal vehicle insurance.
- Background & Records: Successful completion of a criminal background check and a satisfactory motor vehicle record.
- Technical Skills: Proficiency in Microsoft Office (Word, Excel), Zoom, and Microsoft Teams. Experience with Electronic Health Records (EHR) is a plus.
