Join Visiting Angels Columbus West, an award-winning Home Care Company, as a Care Coordinator! We are searching for an additional Care Coordinator to work with our clients to provide quality home care services. This is a challenging but rewarding opportunity for someone who wants to feel like they are making a difference in the life of seniors in Central Ohio by matching caregiver skills/talents with client needs. Must be able to multi task at a high level and be able to "fit" and change schedules together as needed.
Conduct prospect client assessments to determine care needs
Develop and oversee plans of care for clients. Make ongoing revisions as needed.
Serve as an advocate for the client by communicating with other healthcare service personnel and community-based service providers, as delegated.
Perform meet and greets to introduce new caregivers to clients.
Conduct supervisory visits as needed, complete documentation, and update internal records
Schedule caregivers to meet client scheduling and care needs
Conduct regularly scheduled follow up telephone calls and home visits with clients to ensure high satisfaction
Counsel caregivers to meet client and company expectations
Recognize and reward good job performance and promote caregiver recognition
Understand recruitment and retention needs and the existing recruitment market environment and caregiver pool
Perform reference and criminal background verifications for potential employees.
Schedule and complete interviews for potential caregivers applicants
Ensure all new hire documentation (BCI report, TB test results, etc.) is collected, signed off on by the HR Manager and filed in the New Hire file.
Prepare for and facilitate a portion of New Hire Orientation.
Organize and maintain file systems in compliance with office policies and regulations.
Utilizing various social media platforms, ensure robust social media presence to enhance reach of recruitment efforts and engage with potential caregivers and clients.
Timely copying and filing of appropriate caregiver and client documentation.
Develop and maintain positive relationships with clients, their families, and caregivers
Occasional paid on call responsibilities

To be considered:
- Prior nursing, client assessment, training, hiring and/or scheduling experience preferred.
- Nursing degree, LPN or STNA license is a huge plus!
- Ability to listen and communicate clearly fluently and diplomatically.
- Present a well-groomed image that reflects the professionalism of the business.
- Able to remain flexible, resilient, calm and maintain a sense of humor in all situations.
- Work independently and proactively with minimum direction or supervision
- Proficient with Microsoft Office, scheduling systems, and other healthcare industry-related software
- Pass a thorough background screening and have excellent employment references.
