Insurance Coordinator working with CKD population
PURPOSE AND SCOPE:
Supports FMCNA’s mission, vision, core values and customer service philosophy. Adheres to the FMCNA Compliance Program, including following all regulatory and FMS policy requirements. Explores options and coordinates services for late stage CKD patients to ensure a planned transition to in-center FC. Focuses on improving the physiological and psychological health of the CKD patient, to enhance access to care through the coordination of appropriate insurance counseling, and to provide specialized insurance advice for the CKD stage 4 & 5 patients; through each stage of their transition to ESRD that is unique to the patient's’ individualized needs.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Partners with Renal Care Coordinators, Treatment Options Specialists, Nephrology Practices, Hospital & Patient Service Managers, and FMCNA Operations to:
- Identify and address the financial and insurance concerns of CKD patients
- Provide specialized insurance education and assistance to stage 4 & 5 patients, through each stage of their transition to ESRD.
- Attends Treatment Options Classes and/or Education Classes as specified by the Nephrology practice to inform patients of available resources, answer any insurance questions and schedule one-on-one appointments with patients where applicable.
- Accepts all referrals from RCC, TOPs, and Nephrology practice staff, documents communication with patients (CKD Monthly Progress Note and /or Nephrology electronic medical records) and communicates follow-up to referral sources.
- Patient contact can be either face-to-face at the Nephrology practice, Treatment Options Class, or by phone. All contact should be conducted in a confidential location.
- Assists with the transition of care from CKD to ESRD and collaborate with the in-center FC on patient’s insurance status and pertinent insurance needs.
- Completes monthly progress notes and monthly trend reports to document patient contact and track trends.
- Other duties as assigned and identified.

PHYSICAL DEMANDS AND WORKING CONDITIONS:
The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Day to day work includes desk and personal computer work and interaction with patients and facility staff. The work environment is characteristic of a health care facility with air temperature control and moderate noise levels. Extensive local travel to clinics in a specified geographic area; must have a valid Driver's License.
EDUCATION:
- Bachelor's’ degree required – Social Work or other Healthcare focus preferred Or RN associates with two years of nursing experience in lieu of degree.
EXPERIENCE AND REQUIRED SKILLS:
At least 1 year of Healthcare experience with specific knowledge of and experience with Medicare, Social Security and Medicaid systems.
Excellent written, verbal and presentation skills.
Detail oriented.
Strong organizational and time management skills.
Proficient in Excel and Microsoft Word
Valid driver's license
