Your Role:
As a Chronic Care Registered Nurse (Care Manager) at Community Health, you will play a vital role in supporting individuals living with complex and chronic conditions, many of whom face significant barriers to self-care. Your responsibilities will include:
Identifying and recruiting appropriate patients for care management in collaboration with primary care providers.
Meeting patients where they are, without judgment, and providing education on chronic medical conditions and medications.
Using motivational interviewing techniques to promote behavioral change and self-management skills.
Assessing, triaging, and responding rapidly to clinical changes to prevent emergencies.
Conducting medication reconciliation in collaboration with clinical pharmacists.
Engaging members and caregivers in active care planning, focusing on medical, behavioral, and social needs.
Providing care coordination, facilitating transitions, and ensuring appropriate follow-up.
Collaborating with interdisciplinary teams and participating in program planning and growth.
Your Qualifications:
We're seeking candidates with the following qualifications, competencies, and traits:
Bachelor of Science degree in nursing; Massachusetts Registered Nurse license required.
Recent clinical experience in an outpatient medical setting preferred.
Strong interpersonal, communication, and management skills.
Ability to update clinical knowledge through education and review of current literature.
Experience working with historically underserved populations and fluency in multiple languages preferred.
