YOUR RESPONSIBILITIES
Identify and engage patients for care management, working closely with primary care providers.
Meet patients where they are, offering support without judgment.
Educate patients on their conditions and medications, fostering self-management skills.
Utilize motivational interviewing techniques to facilitate behavioral change.
Assess and respond to clinical changes promptly to prevent emergencies.
Collaborate with clinical pharmacists for medication reconciliation.
Engage members and caregivers in active care planning, focusing on holistic needs.
Coordinate care transitions and follow-ups, ensuring seamless support.
Delegate tasks to support staff and provide timely consultation.
Participate in team meetings and contribute to program planning and growth.
Ensure culturally and linguistically appropriate care for all patients.
Maintain confidentiality and adhere to organizational policies and procedures.
YOUR QUALIFICATIONS
Bachelor of Science degree in nursing and Massachusetts RN license required.
Clinical experience in outpatient settings preferred.
Strong interpersonal, communication, and management skills.
Ability to stay updated with clinical knowledge and practices.
Experience working with underserved populations and diverse cultures preferred.
Flexibility to work varying hours and travel between sites as needed.
