We are a Community Health Center committed to providing medical, pharmacy, dental, behavioral health, optical, and vision services to diverse underserved local communities. Annually, we serve a significant number of patients, the majority of whom are low-income and require services in languages other than English.
Position Overview:
As an integral member of the care management team, the Chronic Care RN (Care Manager) will play a pivotal role in improving the lives of individuals managing complex chronic conditions. This position requires flexibility and empathy, often involving outreach efforts that can range from telephonic consultations to in-person visits in various settings such as health centers, communities, homes, or inpatient facilities.
Your Responsibilities:
Identify and enroll appropriate patients into care management programs in collaboration with primary care providers.
Meet patients with empathy and without judgment, understanding their unique challenges and barriers.
Educate patients on chronic conditions, medications, and self-management techniques using motivational interviewing.
Monitor and respond promptly to clinical changes that could necessitate emergency services.
Collaborate with clinical pharmacists for medication reconciliation.
Engage patients and caregivers in active care planning, focusing on holistic healthcare needs.
Coordinate care transitions, referrals, and follow-up care.
Delegate tasks to support staff and ensure timely completion.
Participate in interdisciplinary consultations and team meetings.
Contribute to program planning and growth to meet evolving healthcare needs.
Utilize care management tools effectively to enhance workflow and patient outcomes.
Maintain accurate documentation and adhere to HIPAA standards.
Your Qualifications:
Bachelor of Science degree in nursing; Registered Nurse (RN) license in Massachusetts.
Clinical experience in outpatient settings preferred.
Strong interpersonal, communication, and management skills.
Ability to work independently and collaboratively in a diverse, inter-professional environment.
Commitment to continuous learning and staying updated with clinical practices.
Experience with underserved populations and knowledge of diverse cultures preferred.
Bilingual/bicultural skills preferred.
Familiarity with the Patient Centered Medical Home model is advantageous.
Flexibility to work varied hours and travel between health center locations as needed.
