As a vital member of our care management team, the Chronic Care RN (Care Manager) plays a pivotal role in making a profound difference in the lives of individuals living with complex or chronic conditions. We provide a wide range of services, including medical, pharmacy, dental, behavioral health, optical, and vision services to diverse underserved communities. Join us in serving over 13,500 patients annually, with 90% being low-income individuals, and over 70% requiring services in languages other than English.
RESPONSIBILITIES
Identify and engage appropriate patients for care management in collaboration with primary care providers.
Meet patients where they are, without judgment, and provide education and support in managing chronic conditions.
Utilize motivational interviewing techniques to promote behavioral change.
Assess and respond rapidly to clinical changes, preventing emergencies when possible.
Collaborate with clinical pharmacists for medication reconciliation.
Engage members and caregivers in active care planning, focusing on their unique needs.
Coordinate care transitions and follow-ups, delegating tasks as needed.
Participate in interdisciplinary consultations and team meetings.
Maintain accurate documentation and adhere to quality standards and HIPAA regulations.
Provide assistance in vaccination efforts when required.
Ensure culturally and linguistically appropriate care for all patients.
QUALIFICATIONS
Bachelor of Science degree in nursing and Massachusetts RN licensure required.
Recent clinical experience in outpatient settings preferred.
Strong interpersonal, communication, and management skills.
Ability to work independently and collaboratively within inter-professional teams.
Experience with underserved populations and commitment to diversity, equity, and inclusion.
Flexibility to work flexible hours and travel between sites as needed.
