The position will work to supports patients’ rights through maintaining the confidentiality and processing of protected health information (PHI) in accordance with hospital policies and state/federal laws/regulations. Ensures timely medical record completion in compliance with hospital policies and state/ federal laws/ regulations. Promotes timeliness and quality of medical records completion through an electronic record processing and tracking system. The chart completion process encompasses the electronic health record (EHR) in a high-paced, high-volume chart completion department that monitors physician and mid-level practitioner chart deficiencies and chart completion in regard to ACHC/ CMS regulatory requirements. Follows best practice protocols for authenticating validity of physician orders, regulatory documentation requirements and assuring the integrity of the clinical documentation. (Examples: History and Physicals, Operative Notes, Discharge Summaries.) This requires possession of technical skills for working with an electronic health record (EHR), Onbase, utilizing and maintaining the Epic Chart Completion Tracking system, tracking of chart deficiencies, creating deficiency letters and working with campus Administration/ Leadership and Service Line Leaders to ensure record completion and / or physician suspension. Must be able to work in a fast-paced environment while simultaneously fostering an environment of support and teamwork for assuring a balanced workload and optimal productivity. Must also possess adaptability and flexibility for learning new concepts, skills and dealing with many different situations in a professional and confidential manner. It is also required that the individual can embrace a continuous improvement environment where each member contributes to their own learning as well as partnering with their co-workers.
